Access to episodic primary care: a cross-sectional comparison of walk-in clinics and urgent primary care centers in British Columbia

Aim:
This study aimed to identify publicly reported access characteristics for episodic primary care in BC and provided a clinic-level comparison between walk-in clinics and UPCCs.
Background:Walk-in clinics are non-hospital-based primary care facilities that are designed to operate without appointments and provide increased healthcare access with extended hours. Urgent and Primary Care Centres (UPCCs) were introduced to British Columbia (BC) in 2018 as an additional primary care resource that provided urgent, but not emergent care during extended hours.
Methods:This cross-sectional study used publicly available data from all walk-in clinics and UPCCs in BC. A structured data collection form was used to record access characteristics from clinic websites, including business hours, weekend availability, attachment to a longitudinal family practice, and provision of virtual services.
Findings:In total, 268 clinics were included in the analysis (243 walk-in clinics, 25 UPCCs). Of those, 225 walk-in clinics (92.6%) and two UPCCs (8.0%) were attached to a longitudinal family practice. Only 153 (63%) walk-in clinics offered weekend services, compared to 24 (96%) of UPCCs. Walk-in clinics offered the majority (8,968.6/ 78.4%) of their service hours between 08:00 and 17:00, Monday to Friday. UPCCs offered the majority (889.3/ 53.7%) of their service hours after 17:00.
Conclusion:Most walk-in clinics were associated with a longitudinal family practice and provided the majority of clinic services during typical business hours. More research that includes patient characteristics and care outcomes, analyzed at the clinic level, may be useful to support the optimization of episodic primary healthcare delivery.

Let a discrete group $G$ act on a unital simple $\text{C}^{\ast }$-algebra $A$ by outer automorphisms. We establish a Galois correspondence $H\mapsto A\rtimes _{\unicode[STIX]{x1D6FC},r}H$ between subgroups of $G$ and $\text{C}^{\ast }$-algebras $B$ satisfying $A\subseteq B\subseteq A\rtimes _{\unicode[STIX]{x1D6FC},r}G$, where $A\rtimes _{\unicode[STIX]{x1D6FC},r}G$ denotes the reduced crossed product. For a twisted dynamical system $(A,G,\unicode[STIX]{x1D6FC},\unicode[STIX]{x1D70E})$, we also prove the corresponding result for the reduced twisted crossed product $A\rtimes _{\unicode[STIX]{x1D6FC},r}^{\unicode[STIX]{x1D70E}}G$.

Australian mosquito species significantly impact human health through nuisance biting and the transmission of endemic and exotic pathogens. Surveillance programmes designed to provide an early warning of mosquito-borne disease risk require reliable identification of mosquitoes. This study aimed to investigate the viability of Matrix-Assisted Laser Desorption/Ionization–Time-of-Flight Mass Spectrometry (MALDI-TOF MS) as a rapid and inexpensive approach to the identification of Australian mosquitoes and was validated using a three-step taxonomic approach. A total of 300 mosquitoes representing 21 species were collected from south-eastern New South Wales and morphologically identified. The legs from the mosquitoes were removed and subjected to MALDI-TOF MS analysis. Fifty-eight mosquitoes were sequenced at the cytochrome c oxidase subunit I (cox1) gene region and genetic relationships were analysed. We create the first MALDI-TOF MS spectra database of Australian mosquito species including 19 species. We clearly demonstrate the accuracy of MALDI-TOF MS for identification of Australian mosquitoes. It is especially useful for assessing gaps in the effectiveness of DNA barcoding by differentiating closely related taxa. Indeed, cox1 DNA barcoding was not able to differentiate members of the Culex pipiens group, Cx. quinquefasciatus and Cx. pipiens molestus, but these specimens were correctly identified using MALDI-TOF MS.
Prospective Memory Impairment in Chronic Heart Failure

Although cognitive deficits are common in patients with chronic heart failure (CHF), no study to date has investigated whether these deficits extend to the capacity to execute delayed intentions (prospective memory, PM). This is a surprising omission given the critical role PM plays in correctly implementing many important CHF self-care behaviors. The present study aimed to provide the first empirical assessment of PM function in people with CHF. The key dependent measure was a laboratory measure of PM that closely simulates PM tasks in daily life – Virtual Week. A group comparison design was used, with 30 CHF patients compared to 30 demographically matched controls. Background measures assessing executive functions, working memory, and verbal memory were also administered. The CHF group exhibited significant PM impairment, with difficulties generalizing across different types of PM tasks (event, time, regular, irregular). The CHF group also had moderate deficits on several of the background cognitive measures. Given the level of impairment remained consistent even on tasks that imposed minimal demands on memory for task content, CHF-related difficulties most likely reflects problems with the prospective component. However, exploratory analyses suggest that difficulties with retrospective memory and global cognition (but not executive control), also contribute to the PM difficulties seen in this group. The implications of these data are discussed, and in particular, it is argued that problems with PM may help explain why patient engagement in CHF self-care behaviors is often poor. The life cycle, phenology, and abundance of Epuraea obliquus Hatch was studied near Hinton, Alberta. Most of the life cycle occurs on galls of Endocronartium harknessii (J.P. Moore) Y. Hiratsuka (western gall rust) infecting lodgepole pine (Pinus contorta Dougl. var latifolia Engelm.). Both adults and larvae feed on the spores of the fungus. Individuals of this beetle were found on most galls sampled. Adults overwinter in the soil. They emerge in the spring to seek out and colonize galls. Eggs are laid on the surface of galls, mainly under the periderm, and larvae feed on the fungus, developing through three larval instars. Larvae in the last instar drop from galls to pupate in the soil. Adults leave the soil in late summer and return to feed on inactive galls before overwintering in the soil. The phenology of E. obliquus is closely synchronized with the timing of rust sporulation and the impact of beetle feeding may be an important natural control of western gall rust.

In this paper we prove that, for a type-II1 factor N with a Cartan maximal abelian subalgebra, the Hochschild cohomology groups Hn(N,N)=0 for all n≥1. This generalizes the result of Sinclair and Smith, who proved this for all N having a separable predual.
FETOSCOPIC SURGERY FOR IN-UTERO MANAGEMENT OF CONGENITAL DIAPHRAGMATIC HERNIA

Until recently two possibilities were available to the expectant parents of a fetus diagnosed with a congenital diaphragmatic hernia: termination of pregnancy or continuation of the pregnancy until term with a potential change in the place of delivery. Open fetal surgery has been used to treat a growing number of congenital malformations with life-threatening or highly morbid consequences including congenital diaphragmatic hernia. However, its effectiveness is limited by the occurrence of preterm labour, chorioamniotic membrane separation, preterm prelabour rupture of the membranes and altered fetal homeostasis. These problems were the impetus for the development of minimal access fetal surgery. Developments in endoscopic surgical technology over the past three decades have provided the opportunity to develop techniques adapted for prenatal fetal intervention.
